Essential Functions/Major Responsibilities Regional Community Health Assessment and Improvement Plan: Facilitate the development of a regional Community Health Assessment and Improvement planning process that includes primary and secondary data collection, analysis, community outreach and engagement, prioritization, strategic planning, and implementation of aligned and/or shared strategies.
Facilitate development of shared work plans.
Facilitate consensus around staff-sharing arrangements to carry out the shared regional Community Health Improvement process.
Develop project timelines and work to keep the collaborative on schedule.
Support data collection as part of the regional process.
Work with data analytics staff and other subject matter experts to continuously update and evaluate data collected as part of the Community Health Improvement process and to identify high priority needs.
Provide technical assistance to partners to interpret data and apply it to decision-making.
Provide technical assistance and leadership in development and monitoring of shared Community Health Improvement process strategic plan.
Travel across the region to meet with the PCH partners and to facilitate meetings.
PCH organizational structure development and management: Develop and manage a formal oversight structure as agreed by the PCH partners.
Develop a PCH project budget on a yearly basis.
Work with partners to identify annual reporting needs and divide responsibilities appropriately.
Develop and manage policies related to the PCH.
Attend meetings, and provide minutes, updates, and materials to PCH partners as assigned.
Act as a liaison, and support relationship management efforts through ongoing partner contact.
PCH Website development and management: Maintain the PCH website and conduct quarterly reviews of website content to ensure accuracy, relevance, and readability.
Develop standard operating procedures pertaining to the content, review, and management of the PCH website.
PCH Steering Committee and Workgroup Meeting Facilitation: Convene and facilitate PCH Steering Committee meetings and workgroups.
Co-create structured meeting agendas and provide documented minutes, including key decisions and next steps.
Support partners working on shared or aligned work with capacity-building and facilitation support.
Communicate and respond to internal and external questions and requests maintaining the knowledge necessary to accurately and constructively respond to inquiries.
PCH budget management: Responsible and steward for part of the PCH budget in partnership with the Steering Committee.
Utilizing confidential information to review budget vs actuals on a monthly basis.
Communicate on finances with Steering Committee on a regular basis.
Typical Qualifications Minimum Education Level Masters Specifics: Public health, health systems or equivalent.
Minimum Experience Level Two- Three years Specifics: Systems coordination and facilitation experience with a wide array of partners and organizations.
Substitution Note: Bachelor’s degree in the Public Health field with over 5 years of experience in public health and/or health systems project management.
Driver’s License Oregon Driver's License required Specifics: Must obtain within 30 days of hire Desirable Qualifications: Experience in operationalizing equity in public health programming and across systems.
Knowledge of: Public health and healthcare systems.
Collaboration and cooperation for collective impact.
Facilitation of multiple relationships across various systems.

Work Location: Lincoln County Travel: Moderate travel required to meetings, and multiple site locations across the Linn Benton and Lincoln region.
