Job Description
Job Title: Social Services Case Manager
Job Summary:
We are seeking a dedicated and compassionate Social Services Case Manager to provide support and assistance to individuals and families in need.
The ideal candidate will be responsible for assessing clients' needs, developing care plans, connecting clients to resources and services, and advocating on their behalf.
You will work closely with a diverse range of clients, including at-risk youth, individuals with disabilities, and elderly individuals, to promote their well-being and independence.
Responsibilities and Duties:
- Conduct initial assessments and ongoing evaluations of clients' needs
- Develop individualized care plans and goals for each client
- Connect clients to community resources, such as housing, healthcare, and counseling services
- Advocate for clients' rights and ensure they receive the support they need
- Monitor clients' progress and make adjustments to care plans as needed
- Collaborate with other social services agencies and healthcare providers to coordinate care
- Maintain accurate and up-to-date client records and documentation
- Provide crisis intervention and support during emergencies
Qualifications and Skills:
- Bachelor's degree in social work, psychology, sociology, or a related field
- Previous experience in case management or social services preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of community resources and social services programs
- Compassion and empathy for individuals facing difficult circumstances
- Strong organizational and time-management skills
- Cultural sensitivity and ability to work with diverse populations
