Job Summary The Referral Coordinator receives and processes internal referrals from the care team to outside providers and specialists.
This work includes understanding the medical need documented, action requested by the provider, working with patient requests and insurance companies, sending communication to patients, and clearly and consistently documenting work in the electronic health record.

It is the medical home for over 9,000 patients across five clinics and two counties.
Our clinics are located in Corvallis, Monroe, Lebanon, and Sweet Home.
The CHC provides physical, behavioral, dental care, and medical treatment for substance use disorders.
The Health Centers utilize the clinical team to support patients in accessing health.

The CHC sites are all Patient Center Primary Care Home (PCPCH) certified at a high level.
