Job Summary
We are seeking a Qualified Mental Health Associate (QMHA) - Children's Wraparound Care Coordinator.
Job Summary This position works as a Care Coordinator in the Wraparound program and will provide case management and care coordination services to youth and families with mental health involvement and complex needs in Benton County.
This position is part of a fidelity program and will be responsible for maintaining all fidelity requirements as well as providing accurate and timely documentation.
Responsibilities will include maintaining a caseload of up to 15 youth; providing supportive services with the youth and family in the form of education, resource connecting, and care coordination; facilitating Wraparound team meetings on a regular basis; conducting needs assessments and determining an individualized Plan of Care; finding creative ways to reduce barriers and promote success for our clients; serving as an advocate and liaison for our families to acquire services; and maintaining ongoing collaborative relationships with our valued community partners.
This position will be embedded in the Children and Family Mental Health program and will work as part of an interdisciplinary team.
This position will also be an active participant in the relationships we maintain with our community partners to support the youth and families involved in the program.

Minimum Qualifications The following minimum qualifications are required for this position: Bachelor's degree from an accredited college or university in Psychology, Social Work, Counseling, or closely related field; AND 1 year of related mental health work experience.
Must be registered as a Qualified Mental Health Associate (QMHA) with Mental Health & Addiction Certification Board of Oregon (MHACBO) or be able to obtain certification within 6 months of hire.
An equivalent combination of education and experience may be accepted.
Please note candidates must have at least 6 months of actual work experience to be considered as equivalent.
Special Requirements: Hiring is contingent upon the successful completion of a background check.
Valid Oregon driver's license.
Ideal Candidate: Knowledge of Child-Serving systems in Benton County.
Experience in case management and care coordination.
Experience working with children and families.
Experience and confidence with facilitating meetings.
Strong communication skills.
Strong time management and organizational skills.
