Summary:
The Quality Improvement Program Specialist provides clerical and data support for The Corvallis Clinic’s Quality Improvement department and serves in assisting with both clerical and analytical functions within the department.
Candidates are expected to effectively communicate outcomes and results, engage providers and clinical staff through dynamic presentations, and effectively coordinate meetings, record keeping, and tracking of improvement initiatives.
The position requires a candidate who can effectively work with a diverse group of stakeholders within the healthcare setting.
Principal Responsibilities:
1.
Will participate and maintain a culture within The Corvallis Clinic that is consistent with the content outlined in the Service and Behavioral Standards Handbook.
To this end, employee will be expected to read, have familiarity, and embrace the principles contained within.
1.
Interprets performance improvement data from an organizational perspective and meet with select primary care teams to explain the data to clinical staff and management personnel at Quality Director established cadence.
2.
Assists individual provider teams to facilitate meaningful changes in workflows, when appropriate as directed by the Quality Director.
3.
Interfaces with providers, patients, outside medical groups, and payers as appropriate.
4.
Works within the interdisciplinary team as part of our Patient Centered Primary Care Medical Home (PCPCH) and Primary Care First (PCF) programs.
5.
Engages and empowers patients to participate in the preventative care approach via direct outreach based on patient care gap lists.
6.
Provides support maintaining accurate and up to date information in the EHR by gathering consult notes, diagnoses list, and medication lists.
7.
Performs provider panel management tasks to ensure PCPs are attributed correctly across the EHR, EPIC, and health plan.
8.
Improves the quality of care through continuing education and self-evaluation of the effectiveness of care.
This includes attendance/participation in most in-services/department meetings, continuing education, and remaining current on clinic/department policies and procedures.
9.
Participates in orientation and training of new employees.
10.
Assists in helping to maintain and manages the Quality Newsletter and Quality Education Board.
11.
Assists in answering and handling phone calls in the department.
12.
Occasional work at other Corvallis Clinic Sites as assigned.

Education/Licensure/Experience:
* Associates Degree in Healthcare Related field or Medical Assistant Certification from nationally recognized certification (ex.
CMA, CCMA, RMA, or NCMA) OR at least two (2) years’ experiences in a healthcare setting.
* Experience in the following preferred:
o Care coordination
o Outpatient, ambulatory care and/or community care
o Knowledge of chronic conditions
o Electronic medical record system:
Knowledge and Skills:
* Evidence of working knowledge of the medical home model and other quality payer programs: MIPS, HEDIS, NCQA, etc.
* Demonstrated initiative, ability to work with others in an engaging manner, and good professional judgment.
* Ability to work on multiple tasks simultaneously in a busy, fast-paced environment while maintaining quality of work.
* Knowledge of chronic health conditions.
* Self-motivated.
