If so, we have an exciting opportunity as a Case Manager/Transition Specialist in our residential programs at our Children's Farm Home in Corvallis.
About the Opportunity:
The CM/TS is a member of an interdisciplinary treatment team responsible for collaborating in the provision of aftercare services, discharge planning and supporting therapists with caseload management of clients.
The case management function includes coordinating efforts to ensure all available community resources are considered in developing discharge plans, supporting SSI and DD eligibility applications.
The successful candidate will be mission sensed, seeking meaningful work and career growth.
Have bachelor’s degree in psychology, sociology, education, or related social science field and experience working with children having a variety of needs or a relevant combination of education and experience.
Case management experience and solid verbal and written communication skills are required.
