Summary: The role of the clinic RN is a leadership position.
As a RN you will do a variety of tasks on a daily basis.

Your main focus we be our patients! A majority of your work will be in Chronic and Primary Care Management.

Responsibilities:
You will support our patients with high acuity to live the best lives possible.
You will be creating and updating patient care plans in conjunction with our providers.
You will work with our Medicare population by conducting Annual Wellness Visits in conjunction with our providers, reviewing their history and identifying changes.
Educating them on disease management and healthy lifestyles.
Accurately record pertinent information in the EHR and ensure necessary patient care documentation is processed and done so in a timely manner.
Reach out to our patients after discharge from the hospital or other inpatient facility to ensure discharge needs have been met, reconcile medications, and to coordinate follow up with our providers.
Reviews emergency department utilization and discharge planning daily.
Reviews notes from hospital and communicates issues to healthcare team.
Educate patients about alternatives to the emergency department.
Work with our Remote Monitoring team to help manage that patient population.
Assist with the development and communication of clinical changes in accordance with CMS standards.
Be part of the interdisciplinary team for our Patient Centered Care Medical Home.
Develop and implements new programs as needed.
Work with individual providers to facilitate changes in practice patterns, when appropriate as directed by the Administrator.
Improve the quality of care through continuing education and self-evaluation of the effectiveness of care.
This includes attendance/participation in most in- services/department meetings and remaining current on clinic/department policies and procedures.
Participates in orientation and training of new employees.
Education, Licensure and Experience:
Graduate of an accredited school of registered nursing
One (1) or more years or nursing experience in an acute care hospital and/or in an individual provider’s office or clinic setting.
Current unencumbered State of Oregon license as a Registered Nurse RN required.
BSN is preferred Current Basic Life Support (BLS) certification or ability to complete BLS certification within 90 days of hire required.
Knowledge and Skills:
Evidence of working knowledge of the nursing process.
Adaptability/flexibility time management.
Customer service and the ability to work well both independently and as a member of a multidisciplinary team.
Ability to work on multiple tasks simultaneously in a busy, fast-paced environment while maintaining quality of work.
Knowledge of chronic health conditions and their greater impact.
Self-starter, motivated, and accountable.
Ability to communicate effectively in both written and verbal formats.
Ability to identify complex problems, review information, and navigate to reasonable solutions.
Ability to work well with providers, clinic staff and patients.
