*Wage Scale*: $47.
00/hr - $54.
22/hr *Schedules*: Monday - Friday, 8am-5pm with flexible hours.
Includes weekend on-call rotation.
*Sign-On Bonus*: $7,500 *Benefits Overview*: * *Student Loan Reimbursement*: $3,000 for 3 years post 90 days of employment.

*Position Summary*: The RN Care Manager is pivotal in coordinating patient care and services, promoting quality care through a collaborative process.
This includes facilitating safe transitions to subsequent care settings and complying with all relevant regulations and standards.
*Qualifications*: * Nursing diploma (associate’s or bachelor’s degree in nursing).
* Active RN license in good standing.
* One year of clinical experience in post-acute care setting preferred.
* Experience in case management, utilization review, and discharge planning preferred.
* CCM or RN BC certification preferred.

* Decision-making and knowledge application in accordance with laws and facility guidelines.
