Current unencumbered Oregon credential/licensure in one (1) of the following required: a.
Licensed Clinical Social Worker (LCSW).

Licensed Professional Counselor (LPC).
c.
Licensed Psychologist (PhD or PsyD).
d.
Clinical Social Work Associate (CSWA).
e.
Licensed Master of Social Work (LMSW).
f.
Licensed Marriage and Family Therapist (LMFT).
3.
Five (5) years care management experience that included three (3) years leadership experience required.
