5 hours/week) Position
ESSENTIAL FUNCTIONS:
We are looking for a Community Health Worker candidate who can perform a variety of community
outreach/program recruitment, resource linkages, case management, health education and administrative
office duties.
This position will focus on providing engagement and support services, particularly to families in
our Maternal Child Health Programs.
This position will focus on meeting individuals “where they are at” and
providing non-judgmental, non-coercive services and resources to individuals.
• Excellent public relations, telephone skills and ability to work harmoniously in a team
setting.
• Provides home visits and telehealth visits with focus on care coordination, referral follow up
activities, and parenting education to children/families enrolled in current programs in
accordance with department productivity standards.
Includes case conferencing, related
travel, charting, data entry and billing for eligible services.
• Operate a county vehicle in a safe manner.
• Ability to perform moderately complex clerical, secretarial, and accounting tasks.
• Ability to maintain effective work habits, a calm demeanor in stressful situations, and regular
and predictable work attendance, manage a flexible schedule.
• Knowledgeable of rules and regulations around protected health information.
• Multi-tasking requires data entry for a variety of systems, including complex.

• Chart work to include sorting documents, verifying accuracy, data entry and creating charts.
• Able to use collaborative problem solving and communication skills.
• Proficient with Microsoft Applications such as Excel, and Word
• Ability to assess client income and insurance eligibility.
• Competency to perform vital signs on infants, children, and adults preferred.
• Community Health Worker certification preferred but not required.
MINIMUM QUALIFICATIONS:
High school diploma or GED is required.
Ability to work with minimal supervision.
Education and training
which demonstrates the ability to perform the work described preferred.
Spanish speaking preferred.
Must
have a valid Oregon Driver’s License.
SALARY, BENEFITS, COMPENSATION:
This position is unbenefited part-time.
The current salary range $21.
85-$27.
90 hourly total compensation
depending on qualifications.
