Case Manager RN

Job Title: Case Manager Duties: - Conduct comprehensive assessments of patients' needs and develop individualized care plans - Collaborate with interdisciplinary teams to coordinate and facilitate patient care - Monitor and evaluate patients' progress towards achieving their healthcare goals - Provide counseling and support to patients and their families - Advocate for patients' rights and ensure access to appropriate resources and services - Maintain accurate and up-to-date documentation of patient interactions and interventions - Participate in case conferences and team meetings to discuss patient care strategies Job Type: Contract Pay: $60.
